As part of the Careways renewal process for 2014 we will be revisiting how we allocate work to our affiliates with the aim to provide a more equitable distribution of work across all the regions we serve. We would like to thank all our affiliate partners (you) who support us with timeous submission of all the statutory documents and also those who ensure excellence in treatment planning, execution and reporting.

  • Please upload your latest Indemnity Cover form, your Council registration certificate as well as a recent photo of your practice rooms (If applicable) in the upload boxes below

 

The outcomes in the above areas will also contribute to the 2014 affiliate survey and also align to the fee revision process.
